What treatment options can be used during pregnancy? As discussed in the previous Figure, neither statins, ezetimibe, nor niacin should be used during pregnancy. Colesevelam is considered category B and in animal models colesevelam has not shown fetal harm. However, colesevelam should be used only when clearly needed and when the risk/benefits ratio is clearly favorable for the patient; certainly a woman should have a discussion with her healthcare practitioner before using any drug during pregnancy. As for patients who are pregnant and at very high risk, such as patients with homozygous FH and those with significant atherosclerotic disease, LDL apheresis can be considered.
