Medical Research Council (MRC) Dyspnea Rating Careful: 1-5 Range or 0-4 Range

Fig. 9:  Another common rating scale for dyspnea is a Medical Research Council (MRC) Dyspnea Rating.[5]  This is given as a 1 to 5 range or a 0 to 4 range; Europeans often use 1 to 5 whereas Americans use 0 to 4 scale.  The rating landmarks are the same identifiers, however; the Figure illustrates a 1 to 5 scale, with the lowest level being MRC1 (only getting breathless after strenuous activity) and the highest level of being 5 (too breathless to leave the house, or becoming short of breath upon undressing). 

Notice that this MRC scale is not really asking about dyspnea.  It is actually asking about activity limitation from dyspnea, but people still refer to this as a dyspnea questionnaire.  And again, often in the United States we will have 0 to 4 rather than 1 to 5, but it is the same interval scale.
