Treatment of Intracerebral Hemorrhage: Limited Data for Warfarin

It is important to emphasize that with warfarin, we are very good at reversing the INR, but this may not be the same as actually changing the outcome of the patient once one of these hemorrhages occurs.

Meta-analyses and systematic reviews have shown that there is really no evidence from randomized trials of warfarin therapy that rapid reversal of anticoagulation in patients with an acute anticoagulation-associated intracerebral hemorrhage will improve functional outcome.  I believe that it is important to embrace the concept of functional outcome in addition to reversing the INR.
