Adaptive/Auto Servo Ventilation (ASV)

FIG. 15:  Based on the points reviewed in the preceding Figures, what are the pressure settings for use of ASV? The goal is an EPAP that maintains upper airway patency and treats OSA.  Modern ASV devices come with automatic EPAP support, and the clinician sets the ranges for minimum EPAP (EPAPmin) and maximum IPAP (IPAPmax).  As shown in this Figure, in addition to minimum EPAP, during inspiration the device provides variable inspiratory pressure support (PS).  The minimum inspiratory pressure support (PSmin) is set at 0, so that when ventilation is normal the device operates in a way similar to a CPAP device, but the device has an automatic back-up rate to maintain ventilation during CSA.
