When we talk about the management of AFib, there are two concerns. One is how we are going to deal with the arrhythmia, and that can be either done as a rate control strategy, where we control the heart rate, or as a rhythm control strategy, looking at ways to keep our patients in sinus rhythm. The second thing we have to look at, which is independent of either of these strategies, is stroke prevention. In stroke prevention we have to determine those patients who have a higher risk of stroke and warrant anticoagulation. Treatment of AFib is predominantly driven by the patient’s symptoms, whereas the stroke prevention is based upon the risk factors for thromboembolism.
